West London transition service for young people
Cornock-Taylor is our new West London transition service for young people who are over 16. It offers accommodation and support to vulnerable young people moving towards living independently, including care leavers and homeless young people.
The services comprises two neighbouring houses offering a safe, structured and caring home. This layout enables young people to move easily between high and medium support accommodation as their needs change.
Our higher needs home and step-down home
The higher needs service provides high support with on-site 24 hour staffing and controlled access to the building for residents and visitors.
The step-down service provides high and medium support with staff available 24 hours a day and sleep-in staff on-site overnight.
Like all of our 16+ services we use the Outcomes Star to identify the needs of the young person and how we can help them to create a brighter future. We also have an accredited life skills programme available to our young people.
