For many young people, the transition between life in care and independent existence as a young adult is the most daunting and difficult step they have ever faced.
We have a long history of working in partnership with local authorities to develop, manage and provide high-quality leaving and aftercare services for young people aged 16-21 who are, or have been, in public care.
A process, not an event
We recognise that leaving public care is a gradual process, not a one-off event. The process can have a major social and emotional impact on a young person.
That's why we work in partnership with our young people to assess their individual needs and provide tailored packages of care.
St Christopher's leaving care services
We offer a wide range of support and accommodation for care leavers, depending on their individual needs: from semi-independent supported housing with 24-hour support; to more independent, less intensive specialist provision; to floating support for young care leavers living in their own rented accommodation.
Care leavers aged 16-21 are provided with a programme of support in line with the Leaving Care Act (2000). The aim is to help them develop the confidence and skills needed to live independently. This work is carried out in partnership with the young person's social worker, and focuses on the following areas:
- the level of personal support needed
- the accommodation the young person will live in
- plans for further education
- how to find employment
- the support needed to maintain family and social relationships
- practical skills needed to live independently
- financial and budgeting skills
- meeting health needs
- aftercare support.
Each young person is allocated a personal advisor, whose role and responsibilities include:
- working in partnership with the young person and significant others, to agree and draw up their Pathway Plan
- coordinating preparation for independence
- providing direct support, advice and guidance and keeping in touch until the young person reaches the age of 21
- providing opportunities for young people to explore options and choices
- helping young people maintain and sustain their tenancies.
Care leavers can also use the full range of education and health services that we offer to our residential service users.
